I currently subscribed to the t-mobile family plan. I'm thinking of
leavnig the family plan and switching to another carrier and taking my
number with me. The account is in someone else's name, but t-mobile does
have that number associated with my name. Can I do this? Thanks.

Unless things have changed since I worked as an indirect (independent)
dealer more than a year ago, no. The name, address, social security #,
etc, on both accounts (the one you are porting from and the one you
are porting to) need to match.
