Phone rings aloud even when on vibrate

Maybe you have a clue that can help me.

I usually keep my LG VX 5200 phone on VIBRATE all the time. However,
once in a while when I get call the ring tone goes off.

I'm real careful about keeping it on the VIBRATE setting and not
hitting the side keys.

I called Verizon and they talked me through a reset. However, the
problem has happened twice more in the last two days.

No rhyme or reason as to the caller either. Sometimes it happens when
my wife calls, or one of my kids but sometimes not. It just happened a
couple of minutes ago. All settings still say VIBRATE.

Have you heard of this before? Any ideas?
