Anyone have a copy of the eXplorist USA Mail In Rebate?

The first page is missing the links, it looks like your browser cut off
part of the second one. It works if I put it back together, here is the
whole thing in a wrap proof form.

Both links work fine in browsers other than Firefox (1.5).
