Had a strange experience with Orange

| I bought an Orange pay-as-you-go phone. The sim card of this new phone was
assigned the phone number and balance of my previous sim card.
I decided to set up a PIN code on this new sim card. I went to the
relevant screen and entered the PIN code of my previous SIM card. This
didn't work. Two tries left. Tried it again, just to be sure. Didn't
work. One try left.
So I rang Orange, and they told me to try it again, even though I knew it'd
block it. I presumed they knew what they were talking about, so I tried it
again, and it blocked. An "enter PUK code" screen appeared. They gave me
an unblocking code, which worked, but then a screen came up asking for a
new PIN code. I presumed that all was well and I entered my PIN code of
choice, but then the phone reported that there were nine tries left. The
Orange operator told me to enter it again, and it said eight tries. They
told me to enter a different number, and it just kept counting down.
Then suddenly the "enter PUK code" screen disappeared. The Orange
spokesperson told me I'd have to buy a new SIM card. Luckily, I had a
spare one, and the person was able to transfer my phone number and balance
onto this spare SIM card.
The problem still remains though: I can't set a PIN code on this SIM. Any
idea how?
Also, surely Orange should have been able to help me here? Can anyone make
sense of this (just in case it happens again)?

Changing your SIM card would mean the PIN would of been Orange's
default PIN of 1111 and not your old PIN IIRC.

1111. It's in your guide book somewhere.

The PUK was wrong in that case.

The SIM you refer to in the previous paragraph is now totally dead, it
cannot be revived. The default Orange SIM PIN is 1111.

Somehow you were given the wrong PUK.
